He also voiced his hope that Yerevan State Medical University named after Mkhitar Heratsi will evolve into a regional hub for strengthening professional capacities in research.<\/strong><\/p>\n<p style=\"text-align: justify;\"><strong>In our conversation with the renowned scientist and researcher, we discussed his professional journey, cooperation with the Medical University, innovations introduced by Armenian doctors, and the legacy of Mkhitar Heratsi, one of the greatest physicians and scholars of the 12th century.<\/strong><strong>\u00a0<\/strong><\/p>\n<p style=\"text-align: justify;\"><strong>&#8211; <\/strong><strong>Please, tell a bit about your origins.<\/strong><\/p>\n<p style=\"text-align: justify;\">&#8211; I consider myself a global citizen. A Luxembourg national, I was born in India, studied medicine and public health in Nigeria and the UK, and completed a PhD in operational research in the Netherlands.<\/p>\n<p style=\"text-align: justify;\">I began humanitarian work in the early 1990s, first in Lebanon and then in Somalia. Those experiences sparked my commitment to improving access to health care in disaster settings. There, I met a nurse from Luxembourg; years later, we returned to Luxembourg, which became home.<\/p>\n<p style=\"text-align: justify;\">Over the past three decades, I have worked mainly in conflict zones, natural disasters, outbreaks, and impoverished settings where patients struggled to access treatment for diseases such as HIV\/AIDS and tuberculosis, or where health systems had collapsed. These included Tajikistan, Afghanistan, Rwanda during the 1994 genocide, Kenya, Liberia, and Malawi. I spent more than 20 years with M\u00e9decins Sans Fronti\u00e8res in such contexts.<\/p>\n<p style=\"text-align: justify;\">In the early 2000s, I turned to operational research to improve health-care quality and performance. This led me to the UNICEF\/UNDP\/World Bank\/WHO Special Programme for Research and Training in Tropical Diseases at the World Health Organization, where I am now a scientist and coordinate the global SORT IT partnership. The initiative builds research capacity across 97 countries and numerous institutions, including a new partner, Yerevan State Medical University named after Mkhitar Heratsi.<\/p>\n<p style=\"text-align: justify;\"><strong>&#8211; <\/strong><strong>When did the cooperation with YSMU start?<\/strong><\/p>\n<p style=\"text-align: justify;\">&#8211; We have collaborated with many students from Yerevan State Medical University named after Mkhitar Heratsi over the past decade. A formal partnership to implement SORT IT began in early 2025, and is now a reality. Encouragingly, half of the current mentors were previously trained through SORT IT. Armenian colleagues strongly supported embedding the program within the University to ensure long-term sustainability.<\/p>\n<p style=\"text-align: justify;\">Research and health service delivery often run in parallel, disconnected from each other. The SORT IT model addresses this by integrating research with capacity building. By creating a sustainable system that continuously trains new research graduates, we can ensure a steady stream of professionals generating evidence to strengthen healthcare delivery.<\/p>\n<p style=\"text-align: justify;\">This initiative at YSMU extends beyond Armenia. It has a regional scope, involving participants from Kyrgyzstan, Kazakhstan, and Uzbekistan, with the potential to expand further. The goal is for YSMU to become a regional hub for building the capacity of front-line health workers while fostering closer, mutually beneficial links with academia.<\/p>\n<p style=\"text-align: justify;\"><strong>&#8211; <\/strong><strong>What inspired you to become a scientist?<\/strong><\/p>\n<p style=\"text-align: justify;\">&#8211; Two early experiences in my life shaped my thinking that \u2018research is not a luxury, but a necessity\u2019\u2014it is the science of doing better.<\/p>\n<p style=\"text-align: justify;\">The <strong>first<\/strong> was two decades ago in Mali. During a rural clinic evaluation, patients repeatedly complained that chloroquine was no longer effective. The initial response by clinicians was: \u201cWe are too busy for research.\u201d But studies were eventually conducted and revealed a staggering 95% failure rate in treating Plasmodium falciparum malaria. So, our clinicians were operationally busy\u2014yet providing ineffective treatment.<\/p>\n<p style=\"text-align: justify;\">The findings prompted the World Health Organization to shift to effective therapies, saving countless lives. Without operational research, it would have been business as usual. The lesson was clear: we cannot simply deliver care\u2014we must ask whether it works.<\/p>\n<p style=\"text-align: justify;\">The <strong>second<\/strong> experience came in the 2000s, when sub-Saharan Africa was devastated by HIV\/AIDS. More than 25 million people were infected, and 17 million had died. Amid the despair, Andrew Natsios, then head of USAID, dismissed providing antiretroviral therapy (ART) in Africa, his words were a stunning indictment of both the people and the possibilities:<\/p>\n<p style=\"text-align: justify;\">\u201cThe problems extend to the Africans themselves. They do not know what watches and clocks are&#8230; they don\u2019t know what Western time is, and won\u2019t take these (AIDS) drugs correctly.\u201d<\/p>\n<p style=\"text-align: justify;\">This defeatist rhetoric strengthened my determination to prove otherwise. Through operational research, we demonstrated that universal ART access was feasible and life-saving in heavily affected countries such as Malawi. Patients on the brink of death experienced the \u201cLazarus effect,\u201d regaining health and hope.<\/p>\n<p style=\"text-align: justify;\">In this situation, operational research helped dismantled prejudice, silenced skeptics, and transformed the global HIV response. It showed that effective treatment did not need to mirror sophisticated models in the United States or the United Kingdom\u2014simpler, context-adapted approaches could work. That was when I decided not only to conduct research, but also to train others, using evidence to strengthen advocacy and drive lasting change.<\/p>\n<p style=\"text-align: justify;\"><strong>&#8211; <\/strong><strong>What skills are most important for a scientist and for a research?<\/strong><\/p>\n<p style=\"text-align: justify;\">&#8211; I think, an open mind, and to be always open to observation, to question assumptions, to analyze, and to use these skills to change things for the better.<\/p>\n<p style=\"text-align: justify;\"><strong>&#8211; <\/strong><strong>What\u2019s the most challenging part of the scientific process?<\/strong><\/p>\n<p style=\"text-align: justify;\">&#8211; Once you become a scientist, the focus quickly shifts to publications. Academic careers \u2014 and even world university rankings\u2014are largely measured by the number of papers produced. Publications matter, but what matters more is impact on the ground.<\/p>\n<p style=\"text-align: justify;\">A World Health Organization\u2013led evidence-to-policy summit reported that 86% of applied research is never translated into policy, technology, or practice. In effect, this is research waste.<\/p>\n<p style=\"text-align: justify;\">If research were a transport company or Amazon, such figures would be unacceptable. Of the roughly USD 200 billion invested in research each year, it would mean vast losses in delivery\u2014and many products arriving damaged or unusable. We would call it gross inefficiency, if not fraud, because society is not receiving what it paid for.<\/p>\n<p style=\"text-align: justify;\">The greatest challenge for researchers, therefore, is to ensure that their work benefits populations, advances science, and measurably improves global health.<\/p>\n<p style=\"text-align: justify;\"><strong>&#8211; <\/strong><strong>Could you remember the most exciting project you have worked on?<\/strong><\/p>\n<p style=\"text-align: justify;\">&#8211; It is SORT IT. Why? Because it transformed what I used to do individually into something far more powerful.<\/p>\n<p style=\"text-align: justify;\">Through SORT IT, I can help researchers develop skills, improve health services, address patients\u2019 problems, and foster a culture of inquiry. What began in one country, Malawi, has grown to 97 countries worldwide. Thousands trained through the program have gone on to become leaders in research and mentors to others.<\/p>\n<p style=\"text-align: justify;\">That ripple effect of building capacity that multiplies itself, is the greatest satisfaction of all.<\/p>\n<p style=\"text-align: justify;\"><strong>&#8211; <\/strong><strong>What are your impressions from Armenia and Armenian scientists?<\/strong><\/p>\n<p style=\"text-align: justify;\">&#8211; Armenian scientists are highly innovative and enthusiastic. I saw this firsthand during the COVID-19 outbreak in 2019\u20132020. We had just launched SORT IT in Armenia when the pandemic halted all in-person meetings. Instead of postponing activities, our Armenian colleagues proposed moving the entire training online.<\/p>\n<p style=\"text-align: justify;\">Within six weeks, they developed a comprehensive digital platform for sharing materials, conferencing, and collaborative work. At that time, no such platform for fully online SORT IT training existed anywhere else. Thanks to Armenian doctors, we were the first to implement it\u2014and we have since used it to train participants in 40 countries. The platform is simple, user-friendly, and continues to evolve, with Armenia at its core.<\/p>\n<p style=\"text-align: justify;\">Many Armenian colleagues have also supported trainings abroad\u2014in Africa, Latin America, and Asia\u2014demonstrating strong global engagement.<\/p>\n<p style=\"text-align: justify;\">I see three defining strengths: innovation, openness combined with rapid adaptability, and an exceptional ability to build partnerships. Ultimately, sustainable research depends on people, not projects. Armenia has remarkable people, and many of my international colleagues share this view, so I am not alone in saying it.<\/p>\n<p style=\"text-align: justify;\"><strong>&#8211; <\/strong><strong>Who is a scientist you admire more?<\/strong><\/p>\n<p style=\"text-align: justify;\">&#8211; That is a difficult question. I have read about Hippocrates, Galen, and Mkhitar Heratsi. When I reflect on their legacies, Heratsi stands out to me as different.<\/p>\n<p style=\"text-align: justify;\">He was not only a clinician, but an independent thinker. He did not simply repeat accepted knowledge\u2014he questioned it. What inspires me most is his intellectual courage: the willingness to challenge assumptions and to analyze critically.<\/p>\n<p style=\"text-align: justify;\">His work Consolation of Fevers (1184), addressed to physicians, is considered his masterpiece. In it, he emphasized separating patients with contagious diseases from others\u2014an early form of infection prevention and control, at a time when such thinking was largely absent in much of Europe. This was centuries before microscopes were developed in the late 19th century, yet his ideas anticipated principles later formalized in germ theory.<\/p>\n<p style=\"text-align: justify;\">He was, in every sense, a remarkable scientist. Where we go depends on where we come from. In research, we come from pioneers like Heratsi, Hippocrates, and Galen\u2014individuals who questioned, observed, and laid the intellectual foundations for modern medicine.<\/p>\n<p style=\"text-align: justify;\"><strong>&#8211; What advice would you give to young aspiring scientists?<\/strong><\/p>\n<p style=\"text-align: justify;\">&#8211; First, always observe, question, and analyze\u2014that is the foundation of research.<\/p>\n<p style=\"text-align: justify;\">As you grow as a researcher and begin publishing, do not focus on prestige or high-impact journals. Instead, ask: \u201cWhere are we failing our patients?\u201d. Let that question guide your work so it leads to real impact on people\u2019s lives. What matters is not personal recognition, but how science improves health care.<\/p>\n<p style=\"text-align: justify;\">Second, your legacy should not rest on who you are, but on whom you train. Investing in others multiplies impact far beyond any single publication.<\/p>\n<p style=\"text-align: justify;\">Third, remain open and humble.
